The Benefits of Spinal Manipulative Therapy (SMT)

And why the “click” isn’t the important part
When people think of chiropractic care, they often picture the “click” or “crack” that can happen during an adjustment.
It’s one of the most common questions I get: “Did it work if it didn’t click?”
Short answer… yes.
The sound is not the goal of treatment, and it’s definitely not what makes it effective.
Let’s break down what’s actually going on.

What is Spinal Manipulative Therapy (SMT)?
Spinal Manipulative Therapy (SMT) is a gentle, controlled movement applied to a joint, usually in the spine, to help restore movement and improve how that area functions.
If you’re looking for a chiropractor in South Queensferry, Dalmeny, Kirkliston, Winchburgh, Fife or the surrounding areas of Edinburgh, this is one of the techniques commonly used to help with back pain, neck pain and general stiffness.
Depending on you, this might involve:
A traditional hands-on adjustment
A low-force technique such as Activator
Subtle positioning approaches like SOT
The aim is always the same: to help your body move and function better
So… what is the “click”?
The clicking or popping sound is known as cavitation.
Inside your joints there is a small amount of fluid, and within that fluid are tiny dissolved gases, like oxygen and carbon dioxide. When the joint is moved in a specific way, the pressure changes and those gases quickly form a bubble and release.
That release is what creates the popping sound.
It’s completely normal, harmless, and very similar to clicking your knuckles.
Why the “click” doesn’t matter
It’s easy to assume that a louder or more noticeable click means a better result, but that’s not how it works.
Important things to know:
It doesn’t mean the joint was “out” and is now “back in”
It doesn’t mean the treatment worked better
It doesn’t always happen, even when treatment is effective
Some of the most effective treatments I do don’t involve any noise at all.
What actually matters: how your nervous system responds
Your joints aren’t just mechanical hinges. They are full of tiny sensors that constantly send information to your brain about:
Position
Movement
Tension
This is where things start to make more sense.
What is dysafferentation (in simple terms)?
Dysafferentation is when the messages from your joints to your brain become a bit “noisy” or less accurate.
Think of it like:
A slightly fuzzy radio signal
Or a GPS that’s a bit off
The joint might still move, but the quality of information being sent to your brain isn’t as clear as it could be.
This can happen with:
Stiffness
Injury
Repetitive strain
Long periods in one position
How SMT helps
SMT helps improve the quality of those signals.
Rather than thinking about things being put “back into place”, a more helpful way to look at it is:
We’re improving communication between your body and your brain
This can lead to:
Easier, more comfortable movement
Reduced muscle tension
Better coordination and control
What is proprioception?
Proprioception is your body’s awareness of where it is in space.
It’s what allows you to:
Move without constantly looking at your body
Balance
Coordinate movement smoothly
When proprioception is off, things can feel:
Tight
Unstable
Not quite right
Even if nothing is seriously injured
How SMT improves proprioception
By improving the quality of information coming from your joints, SMT helps your brain get a clearer picture of what your body is doing.
That can mean:
Better movement patterns
Less guarding or over-tension in muscles
More confidence in movement
This is why treatment often feels like “I just move better” rather than just pain disappearing instantly
So what should you expect from treatment?
Instead of focusing on whether something clicked, it’s more useful to notice:
Does movement feel easier?
Does it feel less tight or restricted?
Do things feel more balanced?
That’s the real goal.
The bigger picture
SMT is just one part of what I do.
It’s often combined with:
Soft tissue work
Rehab exercises
Movement and lifestyle advice
If you’re based in South Queensferry, North Queensferry, Dalmeny, Kirkliston, Winchburgh, Fife or surrounding areas of Edinburgh and dealing with back pain, neck pain or ongoing stiffness, treatment is always tailored to you.
Because long-term change doesn’t come from one technique, it comes from helping your body move and function better overall
If you’re not sure if it’s right for you
You’re always welcome to book in for a no-pressure chat first.
We can talk through what’s going on, what your goals are, and whether this approach feels like a good fit for you.
